What this page is
This is a waitlist and intake collection page for Oriva. By submitting your information, you are expressing interest in our services and providing preliminary health information so we can prepare for your care. This is not a subscription, a purchase, or an enrollment in a medical program.
No charges at this time
If you provide a payment method, your card is saved securely through Stripe for future use when our service launches. No charges will be made until you are notified, matched with a clinician, and explicitly authorize a transaction.
Medical disclaimer
The information on this website is for informational purposes only and does not constitute medical advice, diagnosis, or treatment. No provider-patient relationship is created by submitting this form. A provider-patient relationship begins only when you are formally assigned a licensed clinician through the Oriva platform and that clinician accepts you as a patient.
Eligibility
You must be at least 18 years old and a current resident of the United States to use this site and submit the intake form.
Changes
We may update these terms as our service develops. Material changes will be communicated to you via the email address you provide.
